for me, as has mental health conditions and autism. I am lucky enough that my doctor is also on the spectrum. we always enjoy our chats when I see him. but other then that he male sure that I come quiet times and also when I finish up with him he always calls to check how I'm going at the end of the week if I say someone wrong. if I was to prepare myself I use my headphones, figet toys, and have my water, and also I make sure that I'm close to the exit if it's too much.

I write a list of things to talk about when I really need to go. Usually it's about 4 to 6 items long. Then I can go through the items with my doc one at a time at my own pace.
