I'm not on HRT patches, but I do use other patches that are supposed to stay on for 12 hours. I cut a piece of legging and use it over the patch. It's not a perfect solution, but it helps.

Shave the area before you apply it, clean it with soap and water, rinse well, then rub the area down with rubbing alcohol, wait for it to dry. Then, with clean hands, place the patch on the area, press down on it for 20 seconds, then rub down all the edges real good. I wear mine on my stomach and it stays put until I take it off a week later. It's really important to make sure the area is clean and free of lotions or oils, or it won't stick. I shave the area because it hurts to remove if I don't.
